Fecal calprotectin (FCal) is a well-known fecal biomarker that is used to help both distinguish Inflammatory Bowel Disease (IBD) from Irritable Bowel Syndrome (IBS) and to determine whether an IBD patient has an exacerbation. During intestinal inflammation, FCal is secreted by neutrophils in the gut, leading to an increase in FCal levels in fecal samples. While FCal is currently used in patient care, it is far from a perfect test. FCal is degraded in the gut and is therefore not a good predictor of ileal inflammation, which makes it less reliable in CD in which the terminal ileum is commonly inflamed
